Electronic Statement and Notice Disclosure and Agreement

Before you can enroll in or access our Electronic Statement and Notice delivery services ("E-Statement Services"), the Electronic Signatures in Global and National Commerce Act (ESIGN) and other laws and regulations require us to obtain your affirmative consent to electronic delivery of your periodic account statements and notices, and associated disclosures required under Federal and/or State laws or regulations (collectively "E-Statements)"). Please read this Electronic Statement and Notice Disclosure and Agreement (this "Disclosure") carefully and save or print a copy for your records.

Your Consent Is Required

You must consent to electronic delivery of your E-Statement documents in order to continue enrolling in our electronic delivery services. If you do not want to receive your E-Statement documents electronically, you should exit this screen and do not check the box "agree to the terms and conditions" for a paperless statement. If you do not agree to the terms and conditions to receiving your E-Statement documents electronically, our electronic delivery services will not be available to you. If, after reading this Disclosure, you wish to consent to electronic delivery of your E-Statement documents and enroll in our electronic delivery services, please click on "I agree to the terms and conditions" where indicated.

E-Statement Agreement

Definitions: In this Disclosure, the words "you" and "your" mean the owner and/or joint owner of the account who is/are requesting that the E-Statement documents for such account be sent electronically to your personal Internet address and, therefore, is enrolling in our electronic delivery services. The words "Bank," "we," "our," and "us" mean the Central National Bank. Please see the definitions of "E-Statement Services," "E-Statements" and "Disclosure" above.
  1. Agreement Acceptance:

    By clicking "I agree to the terms and conditions" you are agreeing that:
    1. You are requesting that Central National Bank make certain statements, notices, and forms available to you electronically, by sending to your personal Internet e-mail address a notice that your E-Statement documents are available for viewing. These documents can include, but are not limited to, all deposit account statements, disclosures, and notices; Bounce Protection notice; address change notices; tax documents; safe deposit box notices; and statements, records, and documents related to your loan accounts.
    2. If you change your e-mail address, you agree to update that information within Central National Bank Online Mortgage Banking so that we may continue to send you your E-Statement documents electronically. You will also notify us of any changes in physical address or telephone number.
    3. In the event that we are unable to deliver e-mail to you due to an inaccurate e-mail address, your statement or notice may also be accessed through Central National Bank Online Mortgage Banking.
    4. Except as expressly provided in this Disclosure, your participation in the E-Statements Services program does not change your rights and obligations related to your account(s), as amended by us from time to time, and any other agreements and disclosures relevant to your account, including, without limitation, any fees disclosed therein. You agree, however, that when any such terms and conditions, agreement or disclosure includes a time period stemming from your receipt of a Statement from the Bank, such time period shall start or end, as applicable, on the date that we send to your personal Internet e-mail address a notice that your eDoc is available for viewing.
    5. You understand that you have no expectation of privacy if the email address you have provided is not your personal email address. You further agree to release Central National Bank from any liability if the information is intercepted or viewed by an unauthorized party.
  2. System Requirements:

    In order to participate in E-Statement Services and receive and retain copies of your statements and notices electronically, you will need to provide or obtain access to the following: A. Be signed up for Central National Bank Online Mortgage Banking services B. A computer capable of accessing the internet with the ability to download or print agreements and disclosures C. An active "Internet Service Provider" D. A Web Browser that supports 128-bit SSL encryption E. Adobe Acrobat Reader software Version 6.0 or newer
  3. Updating Your Personal Information:

    You must keep us informed of any change in your telephone number, mailing address, or e-mail address. You may notify us of changes in any of the following ways:
    1. Log into Central National Bank Online Mortgage Banking, click the My Loan Tab, then Manage Loan Information and change your personal settings.

  4. Your Rights and Options:

    1. Paper or Non-electronic Form. You are not required to enroll in the E-Statement Services program. You can continue to receive your E-Statement documents in the same format in which you currently receive these items, including, if applicable, paper. If you would like to receive your E-Statement documents electronically, however, you must enroll in the E-Statement Services program and agree to the terms set forth in this Disclosure.
    2. Withdrawal of Consent to Electronic Delivery. If at any time after you consent to electronic delivery of your E-Statement documents (e.g. by clicking "I agree to the terms and conditions"), you wish to withdraw your consent to electronic delivery of your E-Statement documents, you should log into Central National Bank Online Mortgage Banking, click the Account Management tab and then the Document Center tab, and change your E-Statement Services settings. If you withdraw your consent, you will be un-enrolled in the E-Statement Services program and will no longer receive your statements or notices electronically. Statements and notices issued after your withdrawal is effective will be mailed to your physical address of record.
    3. Duration of Consent. If you consent to electronic delivery of your E-Statement documents by clicking "I agree to the terms and conditions", your consent will apply until the earlier of (i) your withdrawal of consent as described in 4(B), above, or (ii) the Bank's discontinuation of electronic delivery services.
    4. Non-Electronic Copy. If, after you are enrolled in the E-Statement Services program and without withdrawing your consent to electronic delivery of E-Statement documents, you would like to obtain a paper copy of a statement or notice you should contact Central National Bank as described in section 3B. The Bank's normal research fees will apply to the generation of paper copies of statements and notices. (Note –It is highly recommended that you save these files to your own computer as soon as possible. The previous 12 statements and the previous 30 days notices will be available to you online. After that time, your account statements and disclosures will be available through normal online account access.)
  5. The Bank's Right to Amend the Services:

    Central National Bank reserves the right to amend E-Statement Services and/or the conditions of participation from time to time, including enlargement or restriction of services available through E-Statement Services. Your use of E-Statement Services after any change will indicate your agreement to the amendments and affirm your consent to electronic delivery of your statements or notices. The Bank will make available to you electronically a copy of any amendments when required by applicable law.
  6. Acceptance of E-Statement Agreement and Consent to Receive Electronic Disclosures:

    Clicking on the "I agree to the terms and conditions" button constitutes your written signature and consent to be bound by the terms of this Disclosure and to receive statements and/or notices electronically. If you do not select the "I agree to the terms and conditions" button, your enrollment in this service will be discontinued, you will not be enrolled, and you will not receive statements and/or notices electronically.
